If you need to surrender a bird, please contact us before bringing the bird to the sanctuary. We operate from a private residence, all visits are by prior arrangement, and our intake capacity is limited.

The form records the bird's history, diet, health, behaviour, routine, belongings and the details we need to provide safe, informed care.
Complete every relevant field, accept the owner-surrender declaration and type your full legal name as your electronic signature. Save the completed PDF and email it to budgie.sanctuary.rescue@gmail.com.

- Contact us firstDo not arrive without an appointment. We will confirm whether we can assist and arrange the next steps.
- Download and completeOpen the PDF in Adobe Acrobat Reader or another app that supports PDF forms, then complete all relevant fields.
- Sign and saveAccept the declaration, type your full legal name as your electronic signature, and save a copy with your name in the filename.
- Email the formAttach the completed PDF and any useful photographs or veterinary records.
Submitting this form does not confirm that we can accept the bird. Budgie Sanctuary Rescue is currently at capacity for permanent aviary residents. Where appropriate, we may discuss assisted rehoming or other options. Never leave a bird at the property without prior agreement.
